Description of forms:

-Direction to Pay, this allows us to be paid directly by your insurance.

-Repair Authorization, this allows us to repair your vehicle.

-Permission to tear down vehicle to assess all related damage

-Permission to allow the vehicle to be scanned by either OEM or Aftermarket software.
-Release of image.

-Removal of personal belongings.

-Financial Responsibility.
